Privacy Policy & SMS Terms

Effective date: November 5, 2026

1. Who we are

Supperkind (“Supperkind,” “we,” “us”) screens New York City Medicaid members for New York State’s Social Care Network (SCN) program and provides nutrition services, such as medically tailored meals, groceries, pantry stocking, and nutrition counseling, to members who qualify. This policy explains how we handle information collected through this website, our phone line, and our text messages.

2. Information we collect

Information you give us

  • Call-back form: your first name, mobile phone number, ZIP code, whether you have Medicaid, preferred language, best time to call, and your call and text consent choices.
  • Screening and services: when you speak with us, we may ask for information needed to screen you and provide services, such as your Medicaid details, household, address for delivery, dietary needs, and health-related social needs.

The 3-question check on our homepage

Your answers to the quick check stay in your browser. They are not sent to Supperkind or stored by us.

Information collected automatically

  • Standard technical data from our web host, such as IP address, browser type, and pages visited.
  • If enabled, advertising and analytics tools (the Meta Pixel and Google tags) that use cookies to measure visits, ad clicks, form submissions, and taps on our phone number. Ad click codes (such as utm, gclid, and fbclid values) may be saved with your request so we know which ad you came from.

3. How we use information

  • To call or text you back about your request, as you asked.
  • To screen you for the SCN program, and to provide, schedule, and deliver food services if you qualify.
  • To send service text messages if you opted in (see Section 5).
  • To meet New York State Medicaid and SCN program requirements, including required reporting.
  • To run, secure, and improve our website, and to measure how well our ads work.

We do not send your form answers or any health information to advertising platforms.

4. How we share information

We do not sell your personal information. We share it only as needed to serve you:

  • Program partners: the regional Social Care Network lead, your Medicaid managed care plan, and New York State agencies, as required to screen, authorize, deliver, and report on services.
  • Service providers: companies that help us operate, such as web hosting, email, phone and texting services, and delivery partners. They may use your information only to provide services to us.
  • Legal reasons: when required by law, or to protect the safety and rights of our clients, staff, or others.

5. Text message (SMS) terms

7. Security and retention

We use reasonable administrative, technical, and physical safeguards to protect your information, and limit access to staff and partners who need it. No method of transmission or storage is completely secure. We keep information as long as needed to provide services and as required by law and program record-keeping rules.
